The first Audi RS 4, codenamed B5, is an ultra-sporty station wagon derived from the A4 Avant, produced from 2000 to 2001 and developed in collaboration with Cosworth, making it a foundational model in the history of modern RS models. It features a 2.7-liter twin-turbo V6 producing 380 hp and approximately 440 Nm of torque, paired with a 6-speed manual transmission and quattro all-wheel drive, enabling a 0 to 100 km/h time of around 4.9 seconds and a top speed limited to 250 km/h.

The RS 4 B5 is distinguished by its widened fenders, specific bumpers, 18-inch wheels, and upgraded brakes, as well as a very sporty Recaro interior with unique instrumentation. The Nogaro Blue paint, already iconic on the RS 2, became one of the most associated colors with the RS 4, its name referencing the French Nogaro circuit, in keeping with Audi's tradition of using circuit names for certain colors (Avus, Imola, Misano, etc.).
